At least in Russia, the difference between this form and the more common Aleksey is possibly that Aleksiy comes directly from Alexios, while Aleksey comes directly from Alexis. Either that, or Aleksiy is an older Russian form of Aleksey. A known bearer of this name is the late 15th patriarch of Moscow (1929-2008), who spelled his name as Алексий (Aleksiy) rather than Алексей (Aleksey). Aleksiy is predominantly used in Russia and other Slavic countries. However, it is also used by Russian-speaking communities around the world.
